Ep 116 Single Pulse Transcranial Magnetic Stimulation (sTMS) in Migraine Treatment with Eric Bohlen, VP Sales at eNeura

Highlights / Quick Pulses from the conversation:

Real-World Feedback and Applications

Well, it's funny. It's been a while since I viewed that video because at the time, that was about seven months ago, I had just transitioned over to eating neuro. I felt that I needed to do this for fellow veterans because I had gotten a lot of questions. And in that seven months, I've learned so much from feedback from doctors like Dr. Williams that you had on here earlier this month, as far as what they're seeing with these migraine patients who were prescribed a Savvy Dual for their migraine. But they're seeing other benefits from it, given that the number of comorbidities, especially with veterans, are numerous. And so we know the technology, TMS technology, especially in clinics with our TMS therapy, is treating behavioral health, mental health. You can Google and see all of the different studies for the different medical indications that they're studying.

The Importance of Alternative Therapies

I get excited when somebody else is aware of TMS therapies because it is such an underappreciated technology just in the general public as a whole because a lot of the attention is sort of sucked up by pharmaceuticals, I believe. And so the more that drive awareness of the additions of, you know, alternative therapies out there, I think just benefits everybody greatly, whether it's hyperbaric chain with oxygen, you know, even acupuncture, transcranial magnetic stimulation. So I think the public as a whole that can derive more benefit from these alternative therapies um i believe it's just a net society good and i i really greatly appreciate the opportunity to be able to come on and just be able to speak about this device we would love if anybody has questions, they're more than welcome to always reach out to me, whether they're a veteran, community based, because I know that a lot of folks haven't seen this, but we do have a lot of clinical research if they are inclined to read it on our website.

Connecting Veterans: Building Support Communities

It's interesting you brought that up because as eNeura grows, we've discussed the ability to create communities so that the veteran community who may be prescribed this device would be able to gather together online. Or maybe we do a newsletter where we can at least relay stories from how it's changed somebody's life. And we do try to connect our clinical nurses who are assigned to each individual patient. They have their own nurse and the nurses do try to network the patients with each other if they did want to discuss. But outside of the VA, in the VA, some VAs actually do have, I guess you would say, support groups that meet time to time. And there is a there's a big transition to a lot of the virtual support groups going on right now. But veteran advocacy, that's actually one of the things that I'm most proud about with the veteran community is it is out there. It's just a matter of tying that all together……..

Exploring Alternative Therapies for Veterans

Um their their depressive symptoms are improving is that because the magnetic pulse is treating the source or is it because they're alleviating the pain from that migraine that's what we'll have to uncover but more importantly a lot of the suicide epidemic in the va is largely due to what they suspect may be over medication. And so the ability to help those patients not burden themselves with medication, especially SSRIs, by using another form or an alternative therapy that's non-invasive, gives the opportunity to treat more holistically that behavioral health. So, yes, long story short, we are looking into funding additional research for this purpose………

Importance of Clinical and Behavioral Health Research

I would love personally to be able to join one of these clinical TMS conferences or societies just to learn from some of the experts in this field, but neurology based. And then when we can go ahead and finish research or at least begin the research findings on what we're seeing on the behavioral health side, being able to join some of those conferences too, and be able to talk about the patients that may have migraine, may have depression, because those patients are out there……….

Non-Invasive Therapies and Veteran Suicide Prevention

Their depressive symptoms are improving. Is that because the magnetic pulse is treating the source or is it because they're alleviating the pain from that migraine? That's what we'll have to uncover, but more importantly, a lot of the suicide epidemic in the VA is largely due to what they suspect may be over medication. And so the ability to help those patients not burden themselves with medication, especially SSRIs, by using another form or an alternative therapy that's non-invasive, gives the opportunity to treat more holistically that behavioral health. Driven by Service: Eric's Commitment to Veterans

I'm actually terrified of heights but i volunteered to become a paratrooper and jump out of airplanes and i spent the rest of my career eight years just uh taking advantage of the opportunities that i knew that i would never have later on in life. But the one thing that always brought me back to the military was having the opportunity to work in the VA with what I always felt were the best people. And that always motivated me. I was always terrified of working in an office. Maybe it was the movie Office Space, I don't know. But I always wanted to be sort of on the go. And my favorite days were being able to walk into a VA and being around the people who I missed back in active duty. And having this opportunity right now, just as far as awareness of the technology of transcranial magnetic stimulation and this particular device, and then meeting all of these veterans online as I network, it motivates me every day to take it to the next level. And just being able to have the chance to be involved with the people who need the best that they deserve and are owed……….

Surprising Benefits of Savvy Dual Therapy

I've learned so much from feedback from doctors like Dr. Williams that you had on here earlier this month, as far as what they're seeing with these migraine patients who were prescribed a Savvy Dual for their migraine, but they're seeing other benefits from it, given that the number of comorbidities, especially with veterans, are numerous. A little more about eNeura:

Our most recent published article looking at 12 month/long term treatment with sTMS:

Single-Pulse Transcranial Magnetic Stimulation for the preventive treatment of difficult-to-treat migraine: a 12-month prospective analysis - PMC (nih.gov)

A team of VA Psychiatrists who are collecting data to determine effectiveness using our SAVI Dual sTMS device for migraine patients and potential secondary mental health benefits.

They regularly share patient successes and hopefully will have something to present to peers later in 2024. Their effort to secure additional funding for a more robust clinical trial is essential.

Based on what they’re observing, hypothesis that patients are experiencing activation of GABA+ in the occipital cortex with continuous use of sTMS pulses for migraine headaches.

And this is directly benefitting the patient’s mental health. Basis for this hypothesis is a 2005 paper:

Clinical application of single‐pulse transcranial magnetic stimulation for the treatment of depression - FUJITA - 2005 - Psychiatry and Clinical Neurosciences - Wiley Online Library

One finding involves a phenomenon known as ‘Olfactory Aura’. Prior to a Migraine attack, a PTSD patient may begin to smell the odor of ‘Death’ and will begin having flashbacks. By using SAVI Dual at the moment they smell ‘Death’, it can end the Olfactory Aura and 1) prevent the Migraine attack and 2) preempt flashbacks and subsequent mental health spiral that can lead to Depression, Anxiety, and Suicidal Ideation.
